Jim Ellison can't explain how Warren Chapel, a permanent patient at a maximum-security mental institution, has detailed firsthand knowledge of a string of grisly murders occurring around Cascade. They quickly discover Chapel was incarcerated for murdering people who were guilty of crimes yet had gotten off for technical reasons, Ellison and Sandberg decide to take a closer look. Sandberg goes undercover as a patient at the hospital and overhears Chapel talking on a secret phone to an outside accomplice. Before Ellison can track down who this accomplice is, however, Chapel escapes. It's up to Ellison to outsmart this deranged killer before he masterminds more of his own twisted brand of lethal justice.
